Why AI adoption stalls

Most organizations adopt AI the same way. A few teams start experimenting, tools spread informally, and leadership is asked to approve something already in motion. Policies get written after the fact, staff are unevenly trained, and nobody is quite sure who is accountable when something goes wrong.
